Traditional (Paper) Youth Application:  BSA Youth Application (Updated 9/19/22)

Online Youth Application: Be A Scout Homepage (Added 9/19/2022)

1. Put in the Mt. Tabor Zip Code 27106
2. Make sure the “Scouts BSA” card is selected (and de-select the “Cub Scouts” card)
3. Hit “Find a Unit”
4. Scroll down to find Troop 0934 Mt Tabor United Methodist (should be #1 or #2 in list)
   Note: Troop 0964 Mt Tabor United Methodist is our sister troop
5. Hit “Apply Now” within the row for Troop 0934 and follow the prompts from there

For youth applications Troop 934 has no preference between the paper or online version.
However, if you elect to use the online application please be sure to let us know to look for it!

BSA Adult Application (Updated 9/19/22)

Medical Form Required for Weekend Campouts

BSA Annual Health Form Parts A, B (Updated 9/19/2022)

• Does not include the medical doctor authorization
• Needed for any and all activities less than 72 hours (3 nights or less)
• Same form is required for adults and youth
• Valid for not more than 365 days (must be renewed annually)
• Must include a photocopy of the Scout or Leader’s insurance card
• Make sure you are using the 2019 Printing (look in the bottom right corner)

Medical/Etc. Required for Summer Camp/High Adventure

BSA Annual Health Form Part A, B & C (Updated 9/19/2022)

• Adds the 1-page medical doctor authorization (Part C), otherwise is the same
• Needed for Summer Camp and other trips in excess of 72 hours (more than 3 nights)
• Same form is required for adults and youth
• Valid for not more than 365 days (expired forms not accepted for Summer Camp/High Adventure)
• Must include a photocopy of the Scout or Leader’s insurance card
• Make sure you are using the 2019 Printing (look in the bottom right corner)

Supplemental Medical-CRK Specific (Added 5/8/23)

• Required for Summer Camp 2023 at CRK

Outdoor Release Form (Added 5/8/23)

• Required for Summer Camp 2023 at CRK
• The form requests a “BSA Registration No.” … if you do not know it leave it blank and we will add it
